Our story
Clear Path Home Health & Care was founded by registered nurse Kimberlee Walton with a clear mission: to deliver the kind of skilled, attentive, and deeply compassionate care she would want for her own family. After years at the bedside, Kimberlee saw how often patients felt lost during the transition home — and how much a steady, knowledgeable presence could change that experience.
As a nurse-led agency, clinical leadership is at the center of everything we do. Our care is created, directed, and overseen by experienced registered nurses, so patients and families receive guidance grounded in real expertise. We believe healing happens best at home, and that the right support can turn an uncertain journey into a clear path forward.
Today we are proud to serve patients, families, physicians, hospitals, and referral partners throughout Central & Southwest Virginia, offering reliable communication and care that families can trust.
