What is skilled home health care?
Skilled home health care brings clinical services — like nursing assessments, wound care, and medication management — into the patient's home. It's designed to help people recover, manage chronic conditions, and stay safe and independent at home, often after a hospital stay or change in health.
Who do you serve?
We care for adults and seniors who need skilled nursing services at home, including those recovering from a hospitalization or managing a chronic condition. We also partner closely with physicians, hospitals, case managers, and discharge planners who need a trusted home health provider for their patients.
Do I need a physician's order or referral?
Skilled home health services are provided under a physician's plan of care. If you don't have an order yet, that's okay — reach out and we'll help guide you and coordinate with your physician to get the process started.
What areas do you serve?
We serve Bedford County, Franklin County, Roanoke, Lynchburg, and surrounding communities throughout Central and Southwest Virginia. If you're unsure whether we cover your area, just ask.
Will my insurance or Medicare cover home health?
Coverage depends on your specific plan and eligibility. Many skilled home health services may be covered when certain criteria are met. We're happy to help you understand your options and verify coverage before care begins.
What's the difference between home health and home care?
Skilled home health is clinical care delivered by licensed professionals such as registered nurses under a plan of care. Home care typically refers to non-medical help with daily activities. Clear Path focuses on skilled, nurse-led home health services.
